CUSTOM LOGO IN SWAROVSKI® CRYSTALS

1. A custom crystal logo can be created for all the headwear, shoe bags, dolly tee purses, tee shirts, tanks, golf towels or totes.
2. Please fax or email artwork/logo to Dolly Mama and we will provide you with a quote within 5-7 business days.
3. Price quotes on logos are approximate and cannot be guaranteed until a sample has been completed by the art department. A set-up charge will apply on all orders that do not meet required minimums.
4. If you are submitting a copyrighted logo, please provide a letter stating that Dolly Mama Designs is legally authorized to reproduce your logo.
5. Designs and Logos shown are for the sole purpose of displaying our creations in Swarovski ® Crystals and are not intended to represent endorsement by individual companies nor are they for sale.
